Subject Access Request

Please complete the form below to lodge a new Subject Access Request. Upon receipt we will process your request and reply within 4 weeks (30 days). If your request has many data points or is complex then we may take an additional 8 weeks (60 days) to respond. If an extension is required we will let you know. We respond to all Subject Access Requests in email format unless you specifically request differently.

Please include the types of data, the dates (to and from) and the names of staff members and/or departments. If you require call recordings please include the number you called from, the dates (to and from) and approximate time.

Identification Document*
Please upload a document to verify your identity. Valid forms are; current UK/EU/EEA passport, driving license, HMRC Tax Notification, Biometric Residence Permit, UK Armed Forces ID Card or Home Office Immigration Status Document

Important Note : All documents must be the most recent ones. We do not accept expired documents. We reserve the right to obtain additional identification and address verification documents if required
